Pulmonary Fibrosis can occur in survivors of pediatric cancer. High frequency oscillatory ventilation (HFOV) and air- way pressure release ventilation (APRV) may be useful in the critical care management of a pediatric patient with Pulmonary Fibrosis

چکیده
In this report we describe difficult ventilation during anesthesia as well as in the pediatric intensive care unit in a pediatric patient with pulmonary fibrosis. We discuss how an initial diagnosis of acute spontaneous pneumothorax, a correctable problem, misdirected perioperative planning and anesthetic management of a patient with severe pulmonary fibrosis.

High-frequency oscillatory ventilation after cardiac surgery: a treatment for all ages
a recent issue of Critical Care, Bojan and colleagues [1] reported their experience with early initiation of high-frequency oscillatory ventilation (HFOV) in pediatric cardiac surgery that demonstrated shorter mechanical ventilation times and intensive care stays. Acute lung injury is common in adult patients undergoing surgery, yet there is scant reporting of the intraoperative utility of HFOV...
متن کاملHigh-frequency oscillatory ventilation and pediatric cardiac surgery: Yes, we can!
In the previous issue of Critical Care, Bojan and colleagues reported their experiences with high-frequency oscillatory ventilation (HFOV) after pediatric cardiac surgery. A total of 120 patients were treated with HFOV on the day of surgery, thus excluding rescue HFOV use. The main finding of the authors was that the duration of mechanical ventilation was significantly shorter in patients in wh...
متن کاملHigh-frequency oscillatory ventilation for acute respiratory distress syndrome.
OBJECTIVE To evaluate the effectiveness of HFOV in pediatric patients with acute respiratory distress syndrome. METHODS In this retrospective study, we reviewed all 20 pediatric patients, who were consecutively ventilated with HFOV in the pediatric intensive care unit of a tertiary medical center, from January 2006 to February 2007. RESULTS A total of 20 patients were enrolled. The median a...
متن کاملPro/con clinical debate: Is high-frequency oscillatory ventilation useful in the management of adult patients with respiratory failure?
In neonatal and pediatric intensive care units, high-frequency oscillatory ventilation (HFOV) has become an increasingly common therapy. This may not have been the case if researchers had not persisted in investigating the therapy after early disappointing clinical trials. Devices capable of providing this therapy to adults have become commercially available relatively recently. However, there ...
متن کاملAirway Pressure Release Ventilation and High-Frequency Oscillatory Ventilation: Potential Strategies to Treat Severe Hypoxemia and Prevent Ventilator-Induced Lung Injury.
Although lifesaving, mechanical ventilation can itself be responsible for damage to lung parenchyma. This ventilator-induced lung injury is especially observed in already injured lungs of patients with ARDS. New ventilatory approaches are needed to safely treat patients with ARDS, and recent studies have suggested the potential utility of open-lung strategies. Airway pressure release ventilatio...
